Below are forms that you can use in your ministry- permission forms, release forms, medical forms. Simply add the name of your parish and the name of your event. In offering these forms we hope to create some consistency across the diocese in protecting our youth and our youth workers.
Medical Form- to be used for each event. We recommend obtaining a copy for each youth in your group. Keep them on file where they can easily accessed if need be. If you hold a parents meeting each year, having a notary handy at the meeting is helpful.
Sample permission/release form
This form should be used for each event in your youth program that involves travel or an overnight stay, or any rigorous physical activity (rock climbing, hiking, swimming etc…)
Transporting Youth form
This form is good to have on file for your youth program. Ask each adult who may be transporting youth to complete it annually and keep them handy.

Healthy and Wholesome policy
We ask that all parishes insure that their Sunday School teachers, youth volunteers and workers have all received the healthy and wholesome policy training. There are a few held each year throughout the diocese. Please contact the diocesan office to find out when and where they are held.
Below is the healthy and wholesome policy and other forms for your adult volunteers to fill out, including a covenant to be signed by each. There is also a form to receive permission to do a background check on each volunteer. The last form is to be used when there has been a suspected incidence of child abuse.

Youth Scholarship Application Form- this form is for all youth of the diocese to use to apply for scholarships for diocesan events, Kanuga, EYE or provincial events. Please fill out the form and mail to Aimee Bostwick at the diocesan office.
